The Kwakiutl District Council Health (KDC Health) provides preventative and Health Promotion Services for 5 Member Nations on northern Vancouver Island.

KDC Health Services covers a broad range of health activities and interventions, including:

  • Communicable Disease Control
  • HIV/AIDS
  • Community Health Nursing
  • Chronic Disease Prevention and Management
  • Pre and Post Natal Care
  • Nutrition
  • Fetal Alcohol Spectrum Disorder mentoring program
  • Arthritis Wellness Program
  • Mental Health Crisis Counseling
  • Addictions Prevention and Health Promotion including supportive councelling pre and post treatment
  • Home and Community Care programs including personal care workers in the home
  • Children's Oral Health Initiative

Leadership Team

  • Implementation of the Health Evaluation 2009 recommendations and the Approval Stratigic Plan
  • The completion of monthly and annual reports
  • Service and/or site teams focus of delivery care
  • Monitoring budgets for service or site program/activities
  • Home and Community Care Program Manager

Professional Practice & Program Delivery Teams

The Professional Practice teams provide support and guidance based on the clinicians' profession and requirements for supervision. There are 3 teams that provide a venue for professional development, supervision and consultation.

  • Nursing Consultant (level 2 from ITHA)
  • Registered Nurse (Community Health Nurse/Home Nursing)
  • Mental Health & Addictions

Community Site Teams

The goal of the Site Team is to provide continuity and consistency of services, site to site and collaboratively amongst team members, while addressing individual and unique community needs.

 

Kwakiutl District Council Health (KDC Health)

The KDC Executive Committee was mandated in 1999 to enter a Health Transfer Agreement with Health Canada to deliver health services to our community members.

We look forward to implementing this partnership and to work collaboratively on a work plan to address the many gaps and barriers our people encounter in accessing health services. It is our goal to eliminate these obstacles so our people receive the equitable health services they need and deserve.
