KDC Health Evaluation Form

KDC Health is working on an Evaluation for their next Health Transfer Agreement. This includes gathering information of core goals and Health programming. KDC Health wishes to continue its evaluation of the Agreement by identifying and measuring outcomes for the Programs and Services. To know what is needed from our community members we have attached two variations of the same form for you to fill out. (one form you can print, or the other you can do online).
